Endorsement final licensure paperwork

A. Official transcripts

You should order 2 official transcripts copies: 1) one to be sent to you and 2) one to be sent to Educational Licensing.

Transcript Request form

The current (2/07) cost for official transcripts is $4 per copy. Make your check payable to PSU. The Registrar's Office address is on the order form. Please send your request and check directly to the Registrar's Office and do not put it in your PSU licensure file.

Making sure all grades are in:
Be sure to check online that all of your grades have been entered before you order your transcripts. If you have not done this before, the directions for looking up unofficial transcripts are below:

    1. First access the PSU website at www.pdx.edu
    2. Click on "Students" (upper left corner)
    3. Click on "Registration and Records"
    4. Click on "PSU Information System"
    5. Enter user ID or social security number
      Example: 123456789
    6. Enter PIN (usually birth date)
      Example: 060650 = June 6, 1950
      NOTE: If you have a login problem, call 503-725-3511, select Option 5
    7. Click on "Student Services and Financial Aid"
    8. Click on "Student Records"
    9. Click on "Final Grades"
    10. Select term from pull-down menu and click "Display Grades"

B. Praxis Scores

All teachers admitted to the Library Media Endorsement program are required to take the Library Media Praxis Test in order for Portland State University to recommend them to the Teacher Standards and Practices Commission for adding an endorsement to their license. Information about the Praxis is listed below. Upon successful completion of this test, be sure to order two official copies of the scores. Have one sent to Portland State (there is a check-off box on the order form) and have the second copy sent to you at your home.

The Library Media Specialist test is designed to measure the knowledge and abilities of examinees who have had preparation in a program for school library media specialists, grades K-12. Because programs in school librarianship are offered at both the undergraduate and graduate levels, the test is appropriate for examinees at either level.

The test content is aimed at the level of knowledge appropriate for the person who is responsible for administering the library media program at the individual school level. The material in the test, therefore, would not be suitable for those in systems with differentiated staffing or those at the district level.
