There are three options to download CE/ED forms.

1. If you are using Microsoft Internet Explorer version 5.5 or newer and have Microsoft Word installed, you can simply click on the Word version to have it open on your computer.

2. If you are not using Microsoft Internet Explorer but have Microsoft Word installed, click on the Word version, download it, and save it to your computer. Check back occasionally for updates. (This will allow you to type within the form.)

3. If you don't have a compatible version of Microsoft Word, choose the PDF (Acrobat) version and print it out. If you need the free Adobe Acrobat reader, click here.
